MoeGo Intake Form is a tool for you to gather information from potential new clients, without offering online booking. 

What's the best use cases of MoeGo Intake Form?

  1. You only allow new clients to submit information or application without requesting appointment. (Intake Form only allows client to fill the form, while online booking allows clients to request appointment with time spots selected.)
  2. You just started MoeGo, either from pen and paper based schedule, or move from a general calendar, you would like to build client profile in MoeGo. 

Simply share your Intake Form link, so your clients can fill and you can save the new profile (including clients and pets details) with just one click!

How to Access:

From MoeGo desktop, or MoeGo mobile app:  Setting > Intake Form

Intake form overview

  1. Intake Form template. Feel free to click into and customize with your own questions. 
  2. Preview: Preview the form before sending to your clients
  3. Share link: Copy the form link, it is a generic link, you can share to any potential clients, you can put on your website, or post onto your social media, or email/text to potential clients.  
  4. Received submission: All of client submissions will appear here.
  5. Create new: You can also create your own intake form

Customize intake form:

1) Questions for pet

  • There are preset attributes of pet. You can choose to Show the question or not;
  • You can choose Required to answer or not. 
  • You can also sort the order of the questions. 
  • The answers to these questions will be saved to pet profile once accepted.
